Dear Doctor,
This question is related to my friend's wife. She is a 39 years old married women and residing in Norway. She is musician and a hard worker. Two weeks back she had Headache mostly in nights and sometimes also got thrombing pain in the tips of the center finger. She met her norwegian doctor they, advised her to take Head X-ray. Doctors didnot tell what they suspect, and what is the problem. Kindly let us know what could be the problem and its probable solutions. My sincere regards & thanks in anticipation !

I appreciate your sympathy for the suffering musician (away in Norway). Does she only sing or play instruments too , as she complains about pain in the fingers too, even as it is paining in the head ?

A distinct possibility may be - sound problem - due to the noise level in the playing field of music, as the decibels decide the intensity of her throbbing head as well as throbbing heart of music lovers. Rarely a hidden neurological ailment may spoil the sport. An X-ray of the head may help to unfathom the sea of troubles by making a headway before all other sophisticated scans may span into the armamentorium of the diagnostic technos.

Query :
I had checked my blood sugar level on 16 Sep 07. Fasting sugar was 142 and sugar pp 169. To rule out possibility of diabetes, I was advised to check my HbA1c and Microalbuminuria. This was done on 20 Sep and the readings are:

HbA1c- 6.6% and Microalbuminuria 8.60 mg/L.

In addition the Mean Plasma Glucose Level(over the past 60 days) was also checked. It was 157.6 mg/dl. Please let me know if I am diabetic and if not what should I do to eliminate the possibility
of becoming diabetic. Please advise.

You are having impaired glucose tolerance - a stage in the life of many diabetics before overtly becoming a diabetic. The present biochemical picture - so nicely depicted by yourself , implies that you need to be careful.

You need to do the following lifestyle changes :

a) Your weight needs to be less than or close to the last two digits of your height(eg : 178 cm).



b) Diet

  • Tie your mouth to tie down the diabetes demon
  • Eat like a pig - but what the pig eats (lots of fibre).
  • Drink like a fish - but what the fish drinks(lots of water).
  • Veg or Non-Veg, don't eat upto the verge of your throat.

c) Exercise

  • The mind to discover self-control.
  • The body to keep below the ideal weight.

d) The legs of the chair - weight,diet,exercise , needs a fourth one - watchful vigilance of your blood chemistry periodically to keep you seated firmly on the health pedestal, keeping your head high and bottoms right.

I am always feeling tense, and have frequent headaches. When I went for a health check-up, I was found to have hypertension. What is the best medicine for blood pressure in the beginning stage. Pl advice.

Answer :

Calm down friend, calm down !! - yes , being not excited on seeing the first reading above(140/90) - and acting like a saint is the first step and the best medication one can take. Other than this, the ideal measures are outlined.

A) Young less than forty - a water pill and a calcium channel blocker.
B) More than forty - a selective beta-blocker ore ACE inhibitor.

It is not as simple as A+B - for many a patient with hypertension - is a sitting bomb, likely to get a stroke, kidney failure, blindness, heart attack or even a terminal appointment with our creator. So the right combination for hypertension depends on age, co-morbid disease, other concomitant medications consumed, habits of the patient - or even the doctor!

Query :
Sir,what is MI? How does it develop?
Couple of yrs back immediately after having my dinner i felt a pinching irritation in my food pipe with no discomfort of any kind. I was subjected to ECG at a local nursing home who declared it to be MI. There after i was advised complete rest and after couple of days i had no other discomfort/pain or guiddiness or perspiration except stomach disorder. My family approached another N.home who admitted me and treated me for having had a minor attack. My question is when i had no symptoms of any pain/sweating/giddiness etc which r normally encounterd - what was it all ?? Nothing can be said about what all has been done in the past but just need ur response to it. Thanx

Answer :

MI - means Myocardial Infarction again meaning damage to heart muscles due to lack of blood circulation to the heart itself, which is filled with blood, supplying the entire body. It is a pity of poverty amongst plenty.

When coronary (blood vessels nourishing the heart) arteries are clogged due to narrowing of blood vessels or a clot develops, this catastrophe puts an apostrophe or even a full stop to life. Lucky you have survived while everyone may not !!! Be Happy, don't worry. Diabetics and people with certain other diseases - may not have the triad of symptoms (pain,sweating or giddiness) as their nerves are benumbed.

Changes in your lifestyle, positive thinking and a little bit of mental exercise even as your body also does it - may avoid another MI by providing enough.

My son is thin and weight is also not enough . Please, tell me what kind of food structures can help him to gain his weight?

Answer :
If your son is not putting on weight the reasons may be due to :

a) Lack of food nutrients - especially proteins & vitamins. Needs protein supplements like adequate quality milk, processed soya etc.

b) Lactose Intolerance - Some children unfortunately cannot acept the protein called lactose in the milk, commonly called milk allergy - due to the lack of certain enzymes to
metabolise milk - Needs to avoid milk.

c) Intestinal Infestations - worms or giardrasis can also cause harm by parasiting i nyour infant's bowels - eating away all the essential nutrients.

The above are the causatives in 90% of children. Some may be due to rare illnesses - which only a thorough physical & pathological screening of the child will reveal.

I'm 31yrs old. My weight has touched 100kgs now, while my height is 5ft 7inches. I'm suffering severe back ache, high Blood Pressure(110/150) and I'm also asthmatic( have breathing disorder). Please guide me how to reduce weight and what should be my eating habits like.I take alcohol and eat non-veg every alternate day. Due to my busy schedule, I'm not able to do any kind of exercise.

Pl suggest me weight reduction effective tips and how to get rid of back ache.

Answer :
For a height of around 170 cms, the ideal weight is 70KGS ONLY. So you are near to 40% overweight. Have pity on your overworking heart(blood pressure), wheezy chest and burdened back.

A. If you can't do physical exercise, at least do some mental exercises to unburden your mind to reduce blood pressure - like yoga & meditation.

B. If your spiritual (indulgence) experiences cannot calm your mind after a cornu-copic diet(NonVeg diet), take a leaf from the Mammoth's(prehistoric elephant) who is a pure vegetarian.

C. You being an asthmatic, ask your doctor whether you are on steroids, which may add to your weight.

D. A diet of only 1000 calories/day.
A walk of 2 kms within 20 mts.
An exercise schedule (Have time - after all , its your body).
Reduce carbohydrates - or high calorie beverages.

and of course - ruling out any physical disease might ease the obese.

What are the most common symptoms of Blood Sugar ? What test(s) do I have to go through to know whether I have got blood sugar or not. Things like froth in the urine and feeling uneasy to control pass of urine make me feel so. Kindly advice.

Query :
I am a regular boozer. I will have 3-4 pegs every time i drink and at least i will drink 3-4 times in a week. I am 96 kgs and 6 ft height. I will do work outs also regularly. My problem is, next day after having the drink, i am feeling droozy like feeling i.e. head is shaking. What is the reason, if i do not drink, this does not happen. I have Hyper tension also but it is in control and i am taking corvidill A-0.5 regularly. Pl don't advice me to stop the drink. What is the alternative.

With 96 kgs in 182 cms with a physique even your enemy will adore, why do you allow it to become sore with HANGOVER - yes, hangover is the aftereffect you feel the morning after your night's indulgence.

As you don't want to stop Alcohol intake, the alternatives are :
-If it is RUM , switch over to Brandy or Whiskey.
-Have a good carbohydrate diet, next day morning and lots of fruits.
-Anyway, you make over your mind whether the hangover is worth the 'spiritual
experience' - in lieu of a spiritual morning.
Have restful nights and zestful mornings.
