We convince ourselves that life will be better after we get married, have a baby, then another. Then we are frustrated that the kids aren’t old enough and we’ll be more content when they are. After that, we’re frustrated that we have teenagers to deal with. We will certainly be happy when they are out of that stage. We tell ourselves that our life will be complete when our spouse gets his or her act together, when we get a nicer car, when we are able to go on a nice vacation, or when we retire. The truth is there’s no better time to be happy than right now.
If not now, when?

Query:
Dear Sir
I got your email and contact via your blog. I am writing to you from Mumbai, and the query is regarding severe pain in the soles of my father Mr. Pankaj Pandya
Currently he is taking Glycomet GP 2 for his diabetes, and is on a relatively modest diet. Plus has hyper tension as well.
His blood sugar levels keep fluctuating from 130-170 (fasting) and 170-220 (post food). However despite the medicines and rest, he suffers from severe pain in both his feet, especially his soles.
The pain is of a constant burning sensation and cramps, which makes it difficult for him to walk or even stand for a short duration. Despite wearing comfortable soft cushioned foot ever, the problem still continues.
I would request you to kindly give your suggestions on the possible causes, and the remedies. And if you have a branch or centre in Mumbai - then we could go there with the detailed reports.
Thanks and regards
Sagar Pandya
Answer:
Good Evening Mr.Pandya,
The sole-stirring problems of your "sweet" father may be due to Diabetes itself causing peripheral vascular disease, compromising blood circulation or Diabetic Neuropathy. The other causes may be Thyroid deficiency, Raynaud's disease, Calcanial Spur to mention a few among the possibilities.
He may need a better control of his Diabetes, Hypertension, PVD, Quitting Smoking (if he smokes) with proper lifestyle changes. He may need a test for Neuropathy (Bio-Thesiometry), Circulation, Vascular Doppler, Vascular Profile (VP) test (read my article), and even angiography. Please consult your Physician for further instructions.
yours truly,
Dr.Ravindranath
Query :
Dear Doctor
I have the below said problems.
1. Pain in the right ankle
2. Pain in the right sole, when I stand for few hours or several minutes.
3. Burning sensation in both the feet ( sole)
4. Numbness started in left foot If I sit for long ie. 2 to 3 hours
5. Burning in the foreskin.
I have undergone following tests. I have family diabetic history.
1. GTT – the results are normal except for 2 hrs result (after 2 hrs it was 137 Normal range is up to 130)
2. Once PP blood sugar was 173 as I have taken more sugar for breakfast.
3. Otherwise my sugar is just between 90 and 110. I have tested it many times.
4. Nerve conduction study (NCS), Neurologist ruled out the problems with nerve but prescribed some medicines for depression ( Serta, Olapin, and zolfresh). I am on these medicines for the past 15 days but for no improvement. Neurologist suggested if there is no improvement I may have to undergo CT scanning of spinal card.
5. Nero Psycologist suggested just to take the above medicines for 2 months and asked me not to undergo any tests / or consultation for 2 months.
6. I hv also taken mecobalamin tablets for a month and no improvement.
7. I hv also taken 3 \4 ME12 injection (500mcg) but for no improvement.
8. I hv also taken 10 – 15 Roff tablets but for no improvement.
I feel that these problems are due to erratic change in blood sugar level.
- Padmanabha, Bangalore
Answer :
Dear Mr.Padmanabha,
Pain in your padham is always due to some strain or sprain, caused by injury or undue exertion or occassionally due to oncoming Arthritis. The leading cause, you already know now, or see an Orthopaedist who has the know-how.
THE SOLE REASON FOR YOUR PAIN IN YOUR SOLE NEED NOT BE DIABETES ALONE. This may have gone wrong with some injury sustained while landing from a step or a vehicle or ocassionally due to a calcanial spur (a small spiky growth in the heel bone, named calcanium), which may need a local anaesthetic treatment. Generally, Diabetics have painless joint diseases, even fractures called charcots arthropathy. Only certain neural involvements cause severe pain. Methycobal injection is not a panacea for all neuropathic problems, foot or elsewhere. Your undulating blood sugar levels indicate more a impaired Glucose intolerance rather than a frank Diabetes.
WEAR A MICROCELLULAR RUBBER FOOTWEAR, OR BEAR A LITTLE MORE, TIME MAY HEAL YOUR HEEL.
yours truly,
Dr.Ravindranath
Query :
I have had hypertension for the last 15 years. i have been taking the recommended drugs for its treatment. however, i find that the use of the drug, alsartan am, disturbs my sleep. what is the antidote to it. I have also been an athlete. I jog or do treadmill for half an hour everyday, followed by some weight training and yoga. all of this does not seem to help reduce my weight which is static at 90kg for the last 15 years.
Of late I also find that when I visit high altitude area I get mild headache and a rise in BP - which was not the case, say, five years ago. At that time I could walk for 5-6 hours at 12,000-14,000 ft without any trouble. In the given situation do you recommend some vitamins and mineral supplements or change of life style - I smoke pipe five to six times a day and get drunk at least twice a week.

Answer :
Dear Mr.Sharma,
ALSARTAN causing all sort of troubles?
May be true in your case. You have to rule out other causes of sleeplessness before changing your medication. At 50 years you have already enjoyed your full quota of smoking for this birth.
You have already smoked your last cigarette.
Your lack of sleeping may be due to midlife blues. Which is common for many a people who attain this age. Your responsibilities as a father to satisfy your grown up children’s demands and needs. As a husband trying to cope with wife needs. As a male you may not rise to the occasion, making vex at sex . As a responsible person you may have to meet many a person with varied situations leading to tension day in and out. Apart from all these your age itself invites ill-health and unmask the distained diseases programmed by the nature.
Some of the above or all of the above or sum of all may cause you sleep disturbances certain pluses and minuses in your life calculations will salvage you from the hopeless situation. You need zestful days and restful nights.
Your feeling of discomfort in the chest may be caused by your chimnified ( carbon deposited ) lungs failing to expand due to loss of elasticity, heart getting ballooned out due to on set of hypertension or gaseous abdomen again caused by your inveterate smoking addiction and accompanying acidity and overwhelming anxiety.
- Quit smoking.
- Have a physical (examination with a doctor)
- Don’t be emotional.

MY WIFE IS A PATIENT OF BRONCHIAL ASTHMA. SHE IS 50 YEARS OLD AND TAKING SALMETER INHALER TWICE A DAY FOR THE LAST 7 YEARS. WILL CONTINUOUS USE OF THIS MEDICINE HAVE LONG TERM EFFECT ON HER HEALTH? IS THERE ANY ALTERNATIVE?
- Bannerjee, Kolkata
Answer :
Flow-halers or Inhalers are excellent devices for asthmatics. As they deliver only metered doses - their long term safety is assured. Be reassured of the relief with the right belief. The drug delivery is minuscule to harm your tissues.

Query :
Dear Doctor,
This question is related to my friend's wife. She is a 39 years old married women and residing in Norway. She is musician and a hard worker. Two weeks back she had Headache mostly in nights and sometimes also got thrombing pain in the tips of the center finger. She met her norwegian doctor they, advised her to take Head X-ray. Doctors didnot tell what they suspect, and what is the problem. Kindly let us know what could be the problem and its probable solutions. My sincere regards & thanks in anticipation !
- Michael, Bangalore-India
Answer :
I appreciate your sympathy for the suffering musician (away in Norway). Does she only sing or play instruments too , as she complains about pain in the fingers too, even as it is paining in the head ?
A distinct possibility may be - sound problem - due to the noise level in the playing field of music, as the decibels decide the intensity of her throbbing head as well as throbbing heart of music lovers. Rarely a hidden neurological ailment may spoil the sport. An X-ray of the head may help to unfathom the sea of troubles by making a headway before all other sophisticated scans may span into the armamentorium of the diagnostic technos.
yours truly,
Dr.Ravindranath

I had checked my blood sugar level on 16 Sep 07. Fasting sugar was 142 and sugar pp 169. To rule out possibility of diabetes, I was advised to check my HbA1c and Microalbuminuria. This was done on 20 Sep and the readings are:
HbA1c- 6.6% and Microalbuminuria 8.60 mg/L.
In addition the Mean Plasma Glucose Level(over the past 60 days) was also checked. It was 157.6 mg/dl. Please let me know if I am diabetic and if not what should I do to eliminate the possibility
of becoming diabetic. Please advise.
- Ramaswamy, Chennai-India
Answer :
'Sweet' friend,
You are having impaired glucose tolerance - a stage in the life of many diabetics before overtly becoming a diabetic. The present biochemical picture - so nicely depicted by yourself , implies that you need to be careful.
a) Your weight needs to be less than or close to the last two digits of your height(eg : 178 cm).
b) Diet
- Tie your mouth to tie down the diabetes demon
- Eat like a pig - but what the pig eats (lots of fibre).
- Drink like a fish - but what the fish drinks(lots of water).
- Veg or Non-Veg, don't eat upto the verge of your throat.
c) Exercise
- The mind to discover self-control.
- The body to keep below the ideal weight.
d) The legs of the chair - weight, diet, exercise , needs a fourth one - watchful vigilance of your blood chemistry periodically to keep you seated firmly on the health pedestal, keeping your head high and bottoms right.

Query :
Sir,what is MI? How does it develop?
Couple of yrs back immediately after having my dinner i felt a pinching irritation in my food pipe with no discomfort of any kind. I was subjected to ECG at a local nursing home who declared it to be MI. There after i was advised complete rest and after couple of days i had no other discomfort/pain or guiddiness or perspiration except stomach disorder. My family approached another N.home who admitted me and treated me for having had a minor attack. My question is when i had no symptoms of any pain/sweating/giddiness etc which r normally encounterd - what was it all ?? Nothing can be said about what all has been done in the past but just need ur response to it. Thanx
- Subramanian, Chennai-India
Answer :
MI - means Myocardial Infarction again meaning damage to heart muscles due to lack of blood circulation to the heart itself, which is filled with blood, supplying the entire body. It is a pity of poverty amongst plenty.
When coronary (blood vessels nourishing the heart) arteries are clogged due to narrowing of blood vessels or a clot develops, this catastrophe puts an apostrophe or even a full stop to life. Lucky you have survived while everyone may not !!! Be Happy, don't worry. Diabetics and people with certain other diseases - may not have the triad of symptoms (pain, sweating or giddiness) as their nerves are benumbed.Changes in your lifestyle, positive thinking and a little bit of mental exercise even as your body also does it - may avoid another MI by providing enough.

Query :
My son is thin and weight is also not enough . Please, tell me what kind of food structures can help him to gain his weight?
- Anamika Ahluwalia, Ahmedabad-India
Answer :
If your son is not putting on weight the reasons may be due to :
a) Lack of food nutrients - especially proteins & vitamins. Needs protein supplements like adequate quality milk, processed soya etc.
b) Lactose Intolerance - Some children unfortunately cannot acept the protein called lactose in the milk, commonly called milk allergy - due to the lack of certain enzymes to
metabolise milk - Needs to avoid milk.
c) Intestinal Infestations - worms or giardrasis can also cause harm by parasiting i nyour infant's bowels - eating away all the essential nutrients.
The above are the causatives in 90% of children. Some may be due to rare illnesses - which only a thorough physical & pathological screening of the child will reveal.

For a height of around 170 cms, the ideal weight is 70KGS ONLY. So you are near to 40% overweight. Have pity on your overworking heart(blood pressure), wheezy chest and burdened back.
A. If you can't do physical exercise, at least do some mental exercises to unburden your mind to reduce blood pressure - like yoga & meditation.
B. If your spiritual (indulgence) experiences cannot calm your mind after a cornu-copic diet(NonVeg diet), take a leaf from the Mammoth's(prehistoric elephant) who is a pure vegetarian.C. You being an asthmatic, ask your doctor whether you are on steroids, which may add to your weight.
D. A diet of only 1000 calories/day.
A walk of 2 kms within 20 mts.
An exercise schedule (Have time - after all , its your body).
Reduce carbohydrates - or high calorie beverages.
and of course - ruling out any physical disease might ease the obese.

I am a regular boozer. I will have 3-4 pegs every time i drink and at least i will drink 3-4 times in a week. I am 96 kgs and 6 ft height. I will do work outs also regularly. My problem is, next day after having the drink, i am feeling droozy like feeling i.e. head is shaking. What is the reason, if i do not drink, this does not happen. I have Hyper tension also but it is in control and i am taking corvidill A-0.5 regularly. Pl don't advice me to stop the drink. What is the alternative.
Kindly suggest me - Ramakrishnan, Chennai
Answer :
Dear 'Bheemboy',
With 96 kgs in 182 cms with a physique even your enemy will adore, why do you allow it to become sore with HANGOVER - yes, hangover is the aftereffect you feel the morning after your night's indulgence.
As you don't want to stop Alcohol intake, the alternatives are :
-If it is RUM , switch over to Brandy or Whiskey.
-Have a good carbohydrate diet, next day morning and lots of fruits.
-Anyway, you make over your mind whether the hangover is worth the 'spiritual
experience' - in lieu of a spiritual morning.
Have restful nights and zestful mornings.
yours spiritually,
